Cost Drivers The business will consider the costs in order to stay in business (Marron, 2014); (Kidder, 2012 ). a) Upfront Costs Since the business is new, there will be several starting costs before  official opening. Upfront costs consist of the purchase of furniture and fittings (table, chairs, coffee machines, installation of EFTPOS (electronic funds transfer at point of sale) and computers, cutlery, and front decoration (Ilasco and Seto, 2012 ); (Spencer, 2010). Advertising expenses will consist of the advertising campaign in the initial year. The advertising drive will be geared towards increasing sales to cater for the other costs (Marquis, Demand Media, 2014). Materials for shop decoration will be used to attract new customers. John Locke and Land Ownership John Locke in The Second Treatise of Civil Government makes several key arguments about what makes land ownable, these ideologies differ from how land ownership works in America but it is easy to see how America’s early days could have aligned with this ideology. In this paper I will focus on two key principles that Locke believed in that are basic requirements for land ownership.The first of these is that land ownership is obtained through labor and that items on the land have no value until labor is applied and the second describes government’s role in land ownership as simply being that the labor applied to land precedes government and government cannot dispose of the estates of the subjects arbitrarily and instead should be limited to securing the life and property of its citizens, and is only necessary because in an ideal, anarchic state of nature, various problems arise that would make life more insecure than under the protection of a minimal state.These two principles allow for the easy identification of claimed lands. While at the same time provide the motivation and encouragement to individuals so that they will want to find land that they can then harness through labor and thus make their own land. It is because of these reasons and more that land ownership through labor is a must for a successful society and a functioning government. In order to ensure that these principles are being viewed and judge from the same sense of meaning a few key words need to be defined as used in this paper.The term land ownership is vital to this paper and its meaning will be defined as, â€Å"the owner of contiguous property that has been improved upon from nature to provide for one’s living†. The term labor will only be used in reference to labor upon an individual’s land and will reference, â€Å"human time and effort put in to a particular task†. The term government will reference only the actua l ruling body that â€Å"influences daily life based on policies and procedures they enact†. At the last term reward will refer to, â€Å"an item or exchange of labor promised or normally expected given in exchange for labor or with no expectation of any kind of return.The principle that labor is a prerequisite to land ownership fits in perfectly with the needs and situation of the early American nation. Outside of key city areas a majority of this nation’s land was unclaimed and unused bearing none of the fruits of labor and essentially leaving all of its value unclaimed and untouched. With a bounty of land larger than anyone person could actually work and apply labor to, it made sense to encourage land expansion. With land expansion individuals could claim land through labor and the value and goods on that land would no longer go to waste as unused items that could have benefitted society as a whole.The second principle helps to place limits on what the government c an do once a parcel of land is owned by an individual. It is safe to say that any reasonable person would not like to see their possessions and livelihood wiped away with one fell swoop, but if governments have no restrictions in the use of their power it would be all too easy to force someone from their land, effectively robbing them of all their possessions and the livelihood that past labor has brought them.This is why Locke insisted that the government role in land owner ship remain limited in scope only to the extent that is needed to ensure this doesn’t happen. Without this assurance from governmental takeover acts similar to his could become standard practice. An unjust government might routinely perform these actions, unjustly claiming the land. This would result in a diminishment of the value of labor and would remove the incentives to work land and would result in a decrease in goods.Because of this looming threat it is clear that government must have some restricti ons on acquiring land to avoid unjust actions and encourage a general sense of trust and security in the government. A government that is without citizens that have some sort of trust and security provided by having a reliable, continual and trustworthy shelter is bound to fail since this is a basic need of humans in general. Modern science can help prove the stress and additional energy expenditure humans endure if they can’t find a sense of comfort, safety and belonging a study performed by Dr.Gilman while at Princeton University showed, â€Å"humans that are in constant fear of loss of shelter expend an average of 2900 calories daily† (Gilman 03/01/2012), along with this extra energy expenditure a lack of shelter creates stress that in turn creates a reduction in productivity. This stress can actually change the human brain reaction method resulting in it , â€Å"going from more sophisticated but slower modes to the faster behaviors of the older brains when we are under stress† (Gilman 03/01/2012).This change in brain behavior limits creative thinking and essentially devolves human thought to a lower state this combined with the increased calories expenditure creates a vast pool of diminished talent that when viewed in a large scale population level, results in a huge waste of resources that could be easily reduced by the assurance of land ownership and security. The easiest way for a government to meet this need is to let individuals provide additional goods to society by applying labor to their land thereby reducing their energy requirements and providing goods for the benefit of all.With all the complications that are possible when it comes to providing for the needs of the many, it only makes to allow individuals the freedom of land ownership that will help reduce their needs while simultaneously benefitting others. This notion of having some sort of fruit from your labor is fundamental to increasing one’s value and having a desire to perform work that can be beneficial to all. A study from Yale University performed by Dr.Greene helps to illustrate just how deeply rooted this concept is into human psychology, an individual will work perform work equivalent to their valuation of a reward. The study was meant to determine the amount of work an individual would perform without a reward versus the amount of work performed for a reward. The study concluded that, â€Å"any individual who was assigned a tasked performed that task to completion with more detail and more efficiently when rewarded as compared to no reward† (Mark et al. 1978).This study helps to demonstrate that human psychology insists that any individual will perform more work and therefore be a more productive member of society if only that individual receives something for the labor they put in. As mentioned earlier if an individual lives under constant threat of having everything taken from them it is essentially making them perform la bor on their land without a reward resulting in less work being performed and less goods for all of mankind, the only solution that removes this fear is placing limits on when and if a government can take someone’s land from them.With removal of this fear an individual can and likely will utilize labor to its fullest extend thereby adding value to the land. It is clear that the benefits of land ownership as defined through the use of labor allows for the creation of the best society possible, one that can help to provide for mankind as a whole, increase the value and abilities of its citizens that actively partake and utilizes the resources at hand to the best of its abilities without waste and undue stress. We lose ourselves in an action packed film about a superhero saving the world from a villain to spending an evening watching soap operas where Mr. Perfect goes through an injustice as Mr. Wrong in Every Sense takes his woman away from him. But before all of this technology took over, this scenery was given to us in a more witty and creative way. We will discuss these mentioned roles in two superb stories that until this day simply cannot be replaced by special effects and/or exaggerated and unnecessary stunts. Let’s begin by discussing this subject along with one of O’Connor’s masterpieces â€Å" A Good Man is Hard to find†. This is a story without restraint on going straight to the point. We may describe O’Connor’s style of writing as a no apologies type of writing. The story begins by portraying an almost ordinary family. There is a grandmother, who plays the main role, along with her son, daughter-in-law, and grandchildren. This family discusses the possibilities of taking a road trip to Florida whereas the grandmother tries her best to persuade them into not following along with the plans. Being the paranoid elder we read about, she brings up the recent release of a killer and saying how â€Å"I couldn’t answer to my conscience† (449) if her family came across him. Moving on to spoiling the finale, the story ends with every member of the family being shot and killed. Although brutal, it is tied completely to the title as the family comes across a cold hearted, merciless killer who did not care for the pleading of poor old grandma. As previously discussed, this is a classic case of the innocent (good) sadly falling under the mercy of a criminal (evil). O’Connor was an author that wrote with a purpose and meaning behind every story. Even though some may argue about this being simple a story for entertainment, we are able to take a look around and notice that scenarios like the one in the story happen around us sadly. Therefore, the reader can relate and surely sympathize for the family and what they went through. One of O’Connor’s greatest attempt in revealing the mindset behind a character like The Misfit is through one of his final words after killing the grandmother by saying â€Å"She would have been a good woman†¦if it had been somebody there to shoot her every minute of her life† (459). Moving along to Boyle’s â€Å"Carnal Knowledge† we will find a variation of the Right vs. Wrong scenario. In this story we will read about the common scenario of a man, Jim, doing whatever it takes with the purpose of pursuing love in the arms of a very selfish and egotistical Alena. Like many, Jim works hard and enjoys eating meat, as he would love nothing more than to have â€Å"Beef, mutton, pork, venison, dripping burgers, and greasy ribs† (569). But his whole world is turned upside down when he meets Alena, as he was laying on the beach feeling lonesome. In contrast to him, Alena is an animal rights fanatic. Regardless, cupid’s arrow had struck Jim from the moment he laid eyes on her. He tried changing everything about himself to try and win her heart. Jim lost his job, became a vegan, and became part of animal rights protests to impress her. The sad part of the story was not love striking Jim. Instead, Alena being as self involved as she was, did not reciprocate the feelings he had for her. She did not reciprocate anything at all for that matter. After all his effort and attempts, Alena ends up with Rolfe and leaving with him to Wyoming, Along with this devastating heart break for Jim, during their whole friendship he was mistreated and being unfairly dealt by the woman who stole his heart. This is another relatable topic as we see this happen all the time around us or to us in our daily life. We find a relationship where there is and abuser and an abused. There is a relationship where there is right vs. wrong. Good vs. evil and right vs. wrong. We learn about this endless battle through stories, poems, movies, etc. Most importantly, we must treasure the brilliant work from authors like Boyle and O’Connor, which show us some of these scenarios through some of their writing. The same way we recognize the faults and the attributes in these characters, we can learn a thing or two and avoid making the same mistakes or catching ourselves while we still can before we hurt someone or even ourselves. We can all agree that right is most important after all.

Influence of Aspirin on Post Tooth Extraction Bleeding ABSTRACT Aim: The aim of the study was to evaluate the influence of aspirin on post extraction bleeding in a clinical setup. Materials and Method: 200 Patients aged between 50 to 65 years, who were indicated for dental extraction was selected from outpatient department of Oral and Maxillofacial Surgery. All the patients were randomly divided in aspirin continuing group (Group A) and aspirin discontinuing group (Group B). After checking all the vital signs extractions were carried out. Bleeding time and clotting time was recorded for evaluation by Chi Square Test. Results: Chi Square test asserted that the bleeding time increased (3.8Â ±0.75) in Group B patients who continued with the antiplatelet therapy while it decreased in Group A patients who discontinued aspirin. Similarly clotting time increased in group B patient and decreased in Group A patient. But in both the groups, bleeding and clotting time remained within normal limits. Conclusion: Assessing most of the dental and medical literature it can be concluded that there is absolutely no need to discontinue anti platelet therapy for any ambulatory dental procedure and even if practitioner wishes to discontinue, it should not be more than 3 days. INTRODUCTION Medical practitioners commonly advice their patients who are on antiplatelet therapy to either stop or alter their medications prior to surgical procedures due to fear of excessive and uncontrolled bleeding. This is a proven fact that aspirin causes increased risk of intraoperative as well as postoperative bleeding and also increased risk of thromboembolic events such as myocardial infraction and cerebrovascular accidents if the drug is continued1. Thrombotic and thromboembolic occlusions of blood vessels are the main cause of ischaemic events in heart, lungs and brain2. In case of blood vessel injury hemostatic mechanism is responsible for stopping the extravasation. Mainly hemostatic mechanism in characterized by two consecutive phases primary and secondary. Primary mechanism arrests early bleeding as a result of platelet plug formation3. Secondary hemostasis phase is mediated by complex cascade of clotting factors which helps in formation of fibrin clot4. In recent years lot of re search and progress have been made in the field of antiplatelet agents and anticoagulants. These drugs have been utilized for the management of arterial thrombosis also2. Even though a number of antiplatelet and anticoagulant agents have been developed, aspirin and warfarin remains the standard drug of choice5. Development of aspirin dates back to 1897 and is considered as one of the safest and cheapest drug worldwide. A general practitioner Lawrence Craven prescribed low dose aspirin (Baby Aspirin) to his 400 patients and none of them developed myocardial infraction6. This was probably the first time in medical history where aspirin was used to prevent myocardial infarction. Since then it has become the drug of choice for cardiologists. The antithrombotic effect of aspirin is mediated by irreversible inhibition of cyclooxygenase activity in platelets. Phospholipase-A2 acts on the cell membrane to release arachidonic acid on activation. Cyclooxygenase acts on arachidonic acid to produce thromboxane A2. Thromboxane A2 is a potent platelet stimulant leading to degranulation of platelet and platelet aggregation. Aspirin inhibits cyclooxygenase enzyme and decreases the level of platelet stimulant thromboxane A2,5 thus increasing the bleeding time. This is the important reason for a medical practitioner to stop aspirin 3 to 7 days prior to any invasive surgery. The purpose of this study was to investigate the influence of aspirin on post extraction bleeding. MATERIALS AND METHOD This study was conducted at the outpatient Department of Oral and Maxillofacial Surgery. Patients who were on aspirin therapy, aged between 50 to 65 years and who had to undergo tooth extraction were selected for the study. Patients on warfarin, non-steroidal anti-inflammatory drugs, heparin, steroids or suffering from blood disorders and diabetes were excluded from the study. Informed consent was obtained from the patients with the ethical committee clearance. Two hundred patients including both males and females whose teeth were indicated for extraction were included in the study. Patients were randomly divided into Group A and Group B of 100 patients each. Group A patients continued to receive aspirin preoperatively while Group B patients were asked to stop aspirin 7 days prior to extraction. Pre operatively all the vital signs (Blood Pressure and Pulse) were measured. Bleeding time (White and Lee technique) and clotting time (Ivy’s technique) was calculated. Extractions were carried out only if the above parameters were within normal range. After tooth was extracted presence or absence of bleeding and bleeding time was recorded. Analgesics and antibiotics were prescribed as needed for pain and infection control. Chi square test was used to evaluate the relative frequencies of patients in both groups. Differences of parametric variables were tested with analysis of variance. RESULTS After applying Chi square test, mean bleeding time was calculated as 2.1Â ±0.52 minutes in the patients who discontinued baby aspirin (Group B) 7 days prior to extraction. Bleeding time of Group A patients who continued aspirin through the entire study was found to be 3.8Â ±0.75 minutes. This difference was statistically significant (p=0.002) [Table 1]. Although there is significant increase in the bleeding time of Group A patients it should be noted that bleeding time of both the groups was within the normal limits. Clotting time of Group B patients was 3.8Â ±0.75 and Group A was 4.7Â ±0.74 which was also within the normal limits (Normal range according to Ivy’s Method: 3 to 5 minutes). DISCUSSION Historically aspirin was used as an anti-inflammatory, analgesic and antipyretic drug for a short period of disease activity. Lawrence Craven in 1950 reported for the first time its long term use to prevent myocardial infarction6. He advocated a lower dose for antiplatelet action. Antiplatelet activity of aspirin occurs at doses ranging as low as 40 mg/day7 to 3208 mg daily. Doses above 320 mg/day decrease the effectiveness of aspirin as antiplatelet agent due to inhibition of prostacyclin production9. However recent clinical trial indicates that 160 mg/day is optimal for antiplatelet action10. In emergencies where urgent antithrombotic action is required a loading dose of 300 mg is advocated9. Usually in United States daily dose of 81 mg, 160 mg or 325 mg are prescribed while in Europe and other countries daily dose of 75 mg, 150mg or 300 mg are prescribed10. Risk of continuing aspirin therapy prior to surgery is that, with the alteration of platelet function longer time period is required to stop the bleeding from a surgical site. This is attributed to the alteration in primary hemostatic mechanism. Burger et al stated that, in patients on aspirin, the average risk of bleeding increases 1.5 folds. At the same time there is a risk in stopping the aspirin prior to surgery leading to a potential risk of rebound of thromboembolic vascular events. On stopping aspirin thromboxane A2 activity increases to a greater extent with decrease in fibrinolytic activity11. Ferrari et al showed the existence of biological platelet rebound phenomenon on interruption of aspirin therapy. This could create a prothrombotic state which may lead to fatal thromboembolic events. Approximately 20% of these episodes are fatal and another 40% can lead to permanent disability12. Practitioners who advocate the stoppage of aspirin have been debating among themselves regarding the time limit to stop aspirin. Literature wise the effect of aspirin on platelets is irreversible. The effect lasts for 7 to 10 days which is the life span of platelets13, 14. Therefore since early days it was recommended to stop aspirin 7 days prior to surgical procedure15-19. Sonksen et al in their study comprising of 52 healthy individuals showed that withdrawal of aspirin for more than 5 days was not recommended20. Wahl et al advocated that aspirin should be discontinued for 3 days only as after 3 days of interruption of aspirin, sufficient number of newer platelets would be present in circulation for hemostasis21, 22. Now again the debate arises whether to stop aspirin therapy or not? Fear for uncontrolled bleeding encourages the practitioners to discontinue the aspirin therapy. Few studies have shown that there is always an increased risk of bleeding in patients continuing aspirin23, 24. Hence few studies recommended stopping of aspirin therapy prior to surgical procedure17, 19, 25. However if the aspirin therapy is discontinued, there is increased risk of thromboembolic events which can be fatal but none of these have been reported in dental literature. Fijnheer et al in his article mentioned that there is scarcity of literature regarding dental surgeries involving patients on aspirin medication26. Little et al recommended that unless bleeding time is increased above 20 minutes aspirin affected platelets would not cause significant bleeding complication22. Similar claims were made by Sonksen et al and Gaspar et al20, 27. Canigral et al conducted a research involving surgical extraction in patients on antithrombotic therapy. In 92% cases bleeding stopped within 10 minutes with pressure alone. This result was in accordance with the present study4. Gaspar et al advocated that ambulatory oral surgical procedures can be performed in patients without discontinuing the use of aspirin27. A recent recommendation from American Heart Association and American College of Cardiology is that, either continuing aspirin or clopidogrel therapy for minor oral surgical procedures in patients with coronary artery stents or delaying treatment until prescribed regimen will be complicated. The present study demonstrated that there was significant increase in the bleeding time in both the groups but in no case it was difficult to stop the bleeding. Although bleeding time increased in the Group A patients but it still remained within the normal range, regardless of whether patients continued or discontinued their aspirin therapy28, 29, 30. This result was similar to the study done by Canigral et al4. Valerin et al conducted a study with 17 patients randomized to aspirin and 19 to placebo and found no differences in bleeding outcomes for patients on aspirin. This finding suggested that there was no need to discontinue aspirin prior to any ambulatory oral surgical procedures31. Adchariyapetch compared the postoperative bleeding on subjects who stopped or continued taking the aspirin for seven days prior to extraction. The mean bleeding time in both the groups was in normal range. After the procedure there was no difficulty in achieving hemostasis. Therefore they concluded that surgical extraction did not require discontinuation of antiplatelet therapy32. Matocha in his study concluded that risk of bleeding after dental extraction is very minimal in the patients with aspirin therapy and did not exceed 0.2 to 2.3%33. Murphy et al concluded with a survey that 86% of the dental practitioners who advised the patient to stop antiplatelet drugs prior to dental extraction did so with the consultation of the patient’s physician and found that the protocol followed by the physicians and dentists was not based on the current recommendations and guidelines34. Napenas et al concluded that the risk of stopping antiplatelet therapy and predisposing the patient to thromboembolic events overweighed the minimal risk of bleeding from dental procedures. This surprised Billy, because the master-at-arms had been nothing but, what seemed to be, nice to him. Throughout the story, Billy witnesses incidents and threats made by Claggart on other members of the ship. This is just the start of tension between Claggart and Billy. "Starry" Vere, the captain of the ship, is a man who believes the military duty of one’s life must rise above the moral values. And everything must be done in a procedure manner. This contrasted Billy’s way of life in which he remains pure and quiet about the mutiny aboard ship. After much turmoil, Claggart decides to approa ch Captain Vere about Billy, accusing him of conspiring to mutiny. Captain Vere is overwhelmed by this information and does not believe what he is hearing. However, to keep things fair and under control he schedules a conference between the three. After hearing the amazing propaganda that Claggart has presented, Billy becomes speechless and strikes Claggart in the head killing him instantly. Captain Vere tries to keep military law before morals and decides to hold a trial in which persuades the jury to sentence Billy to death.
